When the disease is sensitive to melphalan and prednisone , the pain begins to improve within one to two weeks after starting these drugs. As soon as the pain eases, the patient should try walking. Pain caused by holes (lytic lesions) in the vertebrae should be treated with radiation. When x-rays reveal a large lesion in a long bone of the arms or legs, consideration should be given to the insertion of a rod in the hollow center of the bone (intramedullary rod) followed by radiation. The rod stabilizes the bone and prevents a fracture from occurring as bone healing proceeds. Analgesics and narcotics will be required to control acute pain until the above therapeutic measures take effect. • Fluid Intake Increased fluid intake is necessary for patients with Bence Jones protein in the urine. They should drink enough fluids so that they produce approximately 3 L (3 quarts) of urine daily.
